To make your initial visit simpler, we offer health history and registration forms online. This allows you to save time filling out paperwork in office. Prepare the forms prior to your visit, print, and bring with you to your initial dental appointment. We also provide these forms in office if you would like to complete paperwork at the time of your initial visit.
